By booking in for an appointment with Nicky Elliott we assume you accept the following terms & conditions:
All appointments are made in good faith that you will attend, should you need to cancel your appointment you must provide at least 24 hours notice. Failure to do so will incur a 50% cancellation charge. If an appointment is not attended a 100% fee will be charged
Payments for invoices are to be received on receipt of invoice unless otherwise agreed.
Anyone under the age of 18 will need to be accompanied by an adult or legal guardian.

Culinary Medicine is the application of nutrition science in the promotion of health, peak performance and individual care. Practitioners consider each individual to be unique and recommend personalised nutrition and lifestyle programmes rather than a ‘one size fits all’ approach. Practitioners never recommend nutritional therapy as a replacement for medical advice and always refer any client with symptoms that cause concern to their medical professional.

The degree of benefit obtainable from a Culinary Medicine Practitioner may vary between clients with similar health problems and following a similar Culinary Medicine nutritional programme.
Culinary Medicine advice will be tailored to support health conditions and/or health concerns identified and agreed between both parties.
Culinary Medicine Practitioners are not permitted to diagnose, or claim to treat, medical conditions.
Nutritional advice is not a substitute for professional medical advice and/or treatment.
Your Culinary Medicine Practitioner may recommend food supplements as part of your nutritional programme and may receive a commission on these products or services.
Nicky abides by the standards of the FHT, IPHM, CMA, FHTP and the IICT.
The Client understands and agrees to the following:
You (the client) are responsible for contacting your GP about any health concerns.
You give permission for Nicky to contact your GP regarding any agreed aspects of your case.
If you are receiving treatment from your GP, or any other medical provider, you should tell him/her about any nutritional strategy provided by the culinary Medicine Practitioner. (This is necessary because of any possible reaction between medication and the nutritional programme.)
It is important that you inform me about any medical diagnosis, medication, herbal medicine, or food supplements, you are taking as this may affect the nutritional programme.
If you are unclear about the agreed nutritional programme/food supplement doses/time period, you should contact me promptly for clarification.
You must contact me should you wish to continue any specified supplement programme for longer than the original agreed period, to avoid any potential adverse reactions.
You are advised to report any concerns about Nutritional Therapy promptly to me for discussion and action.
We understand the above and agree that our professional relationship will be based on the content of this document. We declare that all the information we share during this professional relationship is confidential and to the best of our knowledge, true and correct.
